As a member of our patients’ coordinated care team, we strive to improve patient outcomes by providing patients with the dialysis medications they need to manage their treatment. We train our pharmacists on the nuances of this complex disease and its impact on a variety of specific end stage renal disease (ESRD) patient populations, including pediatric, geriatric, and diabetic patients, as well as patients with genetic disorders.

Managing ESRD and other chronic medical conditions may require a high quantity of medications for a multitude of symptoms and conditions. This complex medication regimen can be difficult for patients to manage. Our role on the coordinated care team is to help patients understand their medications and possible side effects to support them in following their medication therapy.

Our coordinated care team has numerous interactions with patients during their frequent visits for dialysis treatments. We use these opportunities to remind patients about the importance of adhering to their medication regimen and available support.

Our medication adherence programs help increase patients’ awareness of the purpose and importance of adhering to phosphate binder therapy. By connecting our pharmacy services to the dialysis centers, patients are more successful at adhering to their prescribed medications and demonstrate improved outcomes.

We understand that every renal patient requires individualized care, so we are able to quickly modify our patient’s prescriptions to adapt to his or her changing needs.
